North Macedonia occupies the Vardar Zone, a major suture belt that marks the ancient closure of the Vardar oceanic basin during the Alpine orogeny, and this inherited structure continues to govern the country's seismicity today. Over 200,000 people were left homeless. Smaller damaging events have occurred subsequently, including sequences in the Ohrid region and along the Albanian border.

The tectonic driving force is the extension of the Aegean Province — the broad zone of northward-propagating back-arc extension behind the Hellenic subduction zone — which has rotated the Adriatic block clockwise and opened a series of NW-trending grabens across northern Greece, North Macedonia, and Albania. Lake Ohrid, one of the world's oldest lakes at 3–5 million years, occupies a graben that is still seismically active. North Macedonia applies Eurocode 8 provisions and has invested in building the national seismic monitoring network in cooperation with regional partners.
